Appointments

To schedule an appointment, please call during our regular office hours. Our staff will try to make appointments that best fit your schedule. On the day of your appointment, please bring your current insurance card for our verification purpose. If you are unable to keep your appointment, please give us a 24 hour notice in advance, otherwise you will be charged an appointment cancellation fee.

Payment
All patients are required to pay their co-pay or deductible (if it is not met) at each visit. We accept different forms of payment – cash and all major credit cards.

Returns/Exchanges 

All returns or exchanges must be done within the first 30 days since the day of purchase. No refunds, only exchanges or credit applied to other services/purchases.
